ASSUMPTION OF RISK

I understand that participation in boxing, fitness training, strength training, cardio exercise, sparring, group classes, open gym activities, and related activities at Rayo Boxing & Fitness LLC involves inherent risks including, but not limited to:

- Cuts and bruises

- Muscle strains and sprains

- Broken bones

- Concussions and head injuries

- Physical contact injuries

- Slipping or falling

- Equipment-related injuries

- Serious bodily injury or death

I voluntarily choose to participate in these activities with full knowledge and acceptance of these risks.

RELEASE OF LIABILITY

In consideration for being allowed to participate in activities at Rayo Boxing & Fitness LLC, I hereby release, waive, discharge, and hold harmless:

- Rayo Boxing & Fitness LLC

- Its owners, members, officers, employees, coaches, trainers, volunteers, contractors, and agents

- The property owner/landlord

from any and all liability, claims, demands, damages, rights of action, or causes of action arising out of participation in gym activities, whether caused by negligence or otherwise, to the fullest extent permitted by North Carolina law.

I understand that this waiver includes claims based on ordinary negligence.

MEDICAL ACKNOWLEDGMENT & EMERGENCY CARE

I certify that I am physically capable of participating in boxing and fitness activities.

I understand that Rayo Boxing & Fitness LLC is not responsible for medical evaluations or medical supervision.

In the event of injury or medical emergency, I authorize gym staff to obtain emergency medical treatment if necessary. I understand that I am financially responsible for any medical expenses incurred.

SPARRING & CONTACT TRAINING ACKNOWLEDGMENT

I understand that sparring and contact training involve additional risks of injury, including concussions and head trauma.

I agree:

- To use all required protective equipment

- To follow coach instructions at all times

- Not to spar without coach approval

- To immediately report injuries or symptoms of concussion

No member may participate in sparring without coach approval and proper protective equipment.

Rayo Boxing & Fitness LLC reserves the right to prohibit any member from sparring for safety reasons.

RULES & MEMBER CONDUCT

Members agree to:

- Treat coaches, staff, and members respectfully

- Follow all gym safety rules

- Use equipment properly

- Wear proper workout attire and boxing gear

- Clean equipment after use

- Refrain from fighting, threatening behavior, or harassment

- Avoid reckless or unsafe conduct

Rayo Boxing & Fitness LLC reserves the right to suspend or terminate membership at its discretion.

PHOTO & VIDEO RELEASE

I grant permission to Rayo Boxing & Fitness LLC to use photographs, videos, or recordings of me for promotional, educational, marketing, and social media purposes without compensation.

MEMBERSHIP & BILLING POLICIES

- Membership fees are charged according to the selected membership plan.

- Members may cancel at any time.

- No late fees are charged.

- All payments are final.

- No refunds will be issued for membership fees, training sessions, classes, unused time, missed sessions, or any gym-related purchases.

- Rayo Boxing & Fitness LLC reserves the right to update membership pricing or policies with notice.

PERSONAL PROPERTY

Members understand that personal belongings brought into the gym are at their own risk.

Rayo Boxing & Fitness LLC is not responsible for lost, stolen, or damaged property.

ACKNOWLEDGMENT OF UNDERSTANDING

I acknowledge that:

- I have read and understand this agreement

- I understand the risks involved in boxing and fitness activities

- I voluntarily agree to these terms

- I understand this document affects my legal rights

GENERAL RULES

- Treat all coaches, staff, members, and guests with respect.

- No fighting, bullying, harassment, threats, or aggressive behavior will be tolerated.

- Follow all instructions given by coaches and gym staff.

- Members must conduct themselves safely and responsibly at all times.

- Proper workout attire and athletic footwear must be worn unless instructed otherwise during boxing training.

- Shirts and gloves must be worn during training sessions when required.

- Members must maintain proper hygiene and cleanliness.

EQUIPMENT RULES

- Use equipment properly and safely.

- Return all equipment to its proper place after use.

- Do not intentionally damage gym equipment or property.

- Report damaged or unsafe equipment immediately.

- Clean equipment after use when applicable.

BOXING & SPARRING RULES

- No member may participate in sparring without coach approval and proper protective equipment.

- Protective gear must be worn during approved sparring sessions.

- Unauthorized sparring is strictly prohibited.

- Respect training partners at all times.

- Excessive force, reckless conduct, or unsportsmanlike behavior will not be tolerated.

- Immediately stop training if instructed by gym staff or coaches.

SAFETY RULES

- Members should immediately report injuries or unsafe conditions to gym staff.

- Members experiencing dizziness, illness, pain, or possible concussion symptoms should stop training immediately.

- Drugs, alcohol, smoking, vaping, or illegal substances are prohibited on gym property.

- Weapons of any kind are prohibited.

FACILITY RULES

- No loitering after closing hours.

- Food and drinks should remain in designated areas unless approved by staff.

- Children must be supervised unless participating in an approved class or program.

- Personal belongings are the responsibility of the member.

MEMBERSHIP POLICIES

- All payments are final.

- No refunds will be issued for memberships, classes, training sessions, or gym-related purchases.

- Members may cancel memberships at any time.

- Rayo Boxing & Fitness LLC reserves the right to modify membership pricing, schedules, or policies with notice.

DISCIPLINARY ACTION

Rayo Boxing & Fitness LLC reserves the right to:

- Suspend or terminate memberships

- Deny facility access

- Remove individuals from the premises

for violations of gym rules, misconduct, unsafe behavior, or actions that negatively affect the gym environment or safety of others.

ACKNOWLEDGMENT OF UNDERSTANDING

I acknowledge that:

- I have read and understand the gym rules and code of conduct

- I agree to follow all gym policies and safety requirements

- I understand that failure to follow these rules may result in suspension or termination of membership

MEDIA RELEASE AUTHORIZATION

I grant permission to Rayo Boxing & Fitness LLC and its representatives to photograph, video record, audio record, or otherwise capture my image, likeness, voice, appearance, and participation during gym activities, training sessions, classes, events, sparring sessions, competitions, or related activities.

I understand these materials may be used for:

- Social media content

- Marketing and advertising

- Promotional materials

- Website content

- Flyers and banners

- Educational or training purposes

- Online videos and digital content

USE OF MEDIA

I authorize Rayo Boxing & Fitness LLC to use, reproduce, edit, publish, distribute, or display such media without compensation, royalties, or further approval.

I understand that media content may be publicly visible online or in printed promotional materials.

RELEASE OF CLAIMS

I release and hold harmless:

- Rayo Boxing & Fitness LLC

- Its owners, members, officers, employees, coaches, trainers, volunteers, contractors, and agents

from any claims, demands, or liability related to the use of photographs, videos, recordings, or other media containing my likeness or participation.

NO COMPENSATION

I understand that I will not receive payment or compensation for the use of any media captured by Rayo Boxing & Fitness LLC.

VOLUNTARY AGREEMENT

I acknowledge that:

- I have read and understand this agreement

- I voluntarily consent to the use of my image and likeness

- I understand this authorization remains in effect unless revoked in writing
